Abstract

Objective: Study on ultrasonic characteristics and cytology characteristics through fine needle aspiration of the thyroid nodules in patients with euthyroid Grave’s Methods: cross-sectional descriptive study on 203 euthyroid Basedow patients diagnosed with thyroid nodules on ultrasound, at the Out Patient Department - Bach Mai Hospital. Results: The proportion of patients with multiple nodules was 58.1% and solitary nodule was 41.9%. The percentage of thyroid nodules with the size from 0.5 to 1 cm was the highest, accounting for 37.9%. The mean nodule  diameter  was  1.08  ±  0.82  cm (range 0.27 – 5.02 cm). Thyroid nodules TIRADS 3 accounted for the highest rate of 57.6%, TIRADS 2 accounted for 21.2%, TIRADS 4 accounted  for  18.2%  and 3.0%  was TIRADS 5. The proportion of patients with fine needle aspiration indication was 58.13%. The rate of malignancy was 13.6%, benign accounted for the highest rate of 79.7%, suspected malignancy was 4.24% and 2.46% unspecified. The rate of malignancy in the solitary nodule and multiple nodules was the same (p = 317). However, the benign rate was higher in the multiple nodules group than in the solitary nodule group (p = 0.004). The properties of microcalcifications, irregular margins, taller than wide shape, TIRADS 4, 5 are statistically significantly associated with the rate of malignancy on cytology. Conclusion: The rate of   malignant   thyroid   nodules   in    Grave’s patients was 13.6%. The properties of microcalcifications, irregular margins, taller than wide shape, TIRADS 4, 5 are statistically significantly associated with the rate of malignancy on cytology. Patients with Graves' disease should have periodic thyroid ultrasound for early detection of thyroid cancer.
